Home Health Care Services

You should consider our home health care services if you:

  1. Have been recently discharged from the hospital and are well but not fully recovered
  2. Have recently developed a disease and need help learning to take care of yourself and cope with your illness?
  3. Have a terminal illness and need emotional support and require a program for pain management?
  4. Need help with medications, treatments or dressing changes?
  5. Have limited mobility due to age, illness or injury and need help with daily living tasks?
  6. Prefer to stay at home but need assistance with personal care, light housekeeping and meal preparation?  
  7. Have a family member that is “burning out” trying to look after someone while trying to manage their own life?

Private Care Services

Palliative Care Services

Palliative and in-home hospice care provides holistic management to control pain and other symptoms. It is an ongoing assessment of relief of those symptoms.

Since pain can be physical, spiritual, social and emotional, the hospice team works with existing caregivers, physicians, family and hospital personnel to preserve the integrity of a dying person and family.

When is palliative and in-home hospice care the right choice?

  1. When a patient's condition includes malignant or non-malignant diseases which no longer respond to aggressive curative therapy and the patient’s life expectancy is six months or less.

What events trigger the decision for palliative and in-home hospice care?

  1. The need for palliative management of symptoms such as pain, nausea, vomiting, anorexia, weakness, respiratory distress, bowel obstruction, wounds, etc.
  2. Diminishing results from chemotherapy or radiotherapy.
  3. Family concern and fatigue over responsibility for the caring of a terminally ill family member.
  4. Upcoming hospital discharge with a need for home care.
  5. Patient and family decision to forego further additional procedures or hospitalizations.

When does one begin to consider a "supportive care network" ?

  1. You should begin thinking about it when the terminally-ill patient is still alert, able to function and enjoy daily living. Be prepared for the inevitable.

When does a palliative patient and family need intensive in-home care services?

The patient does not have to be home bound to receive home care service at a hospice since the facility cannot provide constant bed-side assistance and attention on a 24-hour basis.
